STEP 1:  REGISTER

FOR FIRST TIME ATHLETES:

• CREATE AN ONLINE ACCOUNT AND REGISTER by clicking HERE
• Please follow the prompts to create an account.
• Click on Winter Sports Registration and fill in the requested information

FOR RETURNING ATHLETES:

• USE YOUR ACCOUNT ID AND PASSWORD UPDATE by clicking HERE
• Your registration will auto populate.
• Click on Winter Sports Registration and select the new season sport.           

STEP 2:  PHYSICAL & RECHECK FORMS

Sports Physical 

Next, use this Sports Physical link to download and print your sports physical.  All 4 pages must be completed and signed by your doctor or medical provider. This only needs to be completed once a year.  

Return to the Nurse before the   June 15th deadline.

Recheck Form 

Next, use this Sports Physical Recheck link to download and print your recheck form.  This must be completed and signed by the student athlete's parent/guardian. This needs to be completed each season.

STEP 3:  CONCUSSION IMPACT TESTING       

• Student athletes must have ImPact baseline testing every 2 years.  Our Athletic Trainers and Coaching Staff will coordinate all ImPact testing for new and returning athletes.
